Dislocated hip

  • needs to be reduced as soon as possible because of post traumatic AVN
  • with posterior dislocations, reduction also relieves sciatic nerve traction
  • assessment of stability following posterior hip dislocation:
    • assessment of stability following posterior hip dislocation:
    • CT scan
      • < 25% of posterior wall is #’ed = stable
      • > 40 to 50% of the wall is #’ed = unstable
  • # femur
    • in every femoral shaft #, stability of the knee ligaments must be assessed
    • exchange nailing (for an external fixator) is relatively safe if done < 3 weeks
